BAA Template for Business Associate to use with their Business Associates

This agreement is a Business Associate Agreement for use by a Business Associate for distribution and signing by their subsequent Business Associates or subcontractors. This agreement is not intended for a Covered Entity to utilize with an initial Business Associate. This agreement has been updated to include the requirements of the HITECH Act. The agreement is in a Word document format, and will require minimal editing including inserting the legal names of the initial Business Associate, that is, the purchaser of this template, and the subsequent Business Asssociate, that is the business partner of the primary Business Associate, and the entry of certain contact information.
